One of the special conditions of probation is to submit to a urine screen at your expense.  The cost of your urine screen is Forty-Five dollars ($45.00).  This cost is payable by money order only to Comet / Ascension Medical Clinic.

 
   
 Drug Screens will be held on Friday, July 23, 2010. 
   
 

Your instructions are:

 
 
1.Visit my office, Friday, July 23, 2010, between 8:30 am and 3:30 pm to receive a blank Chain of Custody form.

2.Bring the blank Chain of Custody form, a picture I.D., $45.00, and give urine screen at:

Failure to comply with the terms of your probation will result in your probation being revoked and your jail sentence reinstated. Your monthly supervision fee of $25 may be paid at this time by cash or money order made payable to Ascension Parish Probation.

 
   
 

ATTENTION:  Effective immediately, if you do not appear on the date listed above, your probation will be revoked and your jail sentence will be reinstated.


Furthermore, you are expected to fully comply with all inspection procedures at the Medical Clinic prior to your urine screen.  Failure to do so will result in the issuance of a probation warrant for your immediate arrest.
